Our organization offers billing for Medi-Cal Community Health Workers (CHWs), and Peer Support Specialists, as well as training and services related to Enhanced Care Management (ECM), Community Supports (CS),

and Justice-Involved (JI) services.

Community HUB

A Community Hub is not just a service location—it is a catalyst that shifts power toward the people most impacted. It cultivates shared responsibility and collective well-being by embedding residents in real governance: setting priorities, holding partners accountable, leading initiatives, and sustaining continuous feedback on what is working, what is missing, and what must change.

This is accomplished through clear resident leadership pathways, ensuring the Hub remains responsive, credible, and rooted in local priorities rather than external assumptions.

How It Works: Individual Medi-Cal Services

Medi-Cal is California’s Medicaid program

Medi-Cal is California’s Medicaid program, administered by the California Department of Health Care Services (DHCS), that provides health coverage to eligible low-income individuals and families, including children, pregnant people, seniors, and people with disabilities.

Enhanced Care Managment (ECM)

Enhanced Care Management (ECM) is a Medi-Cal service that gives members extra, hands-on help to stay connected to the care and resources they need. Instead of leaving someone to figure everything out alone, ECM pairs the member with a support team—often led by a care manager—who helps organize appointments, coordinate between providers, and connect the member to community-based supports that make daily life more stable. ECM is built around a consistent set of supports (like outreach, planning, coordination, transitions, and family support) so the member has one clear “hub” of help and a more reliable path to follow.

Justice Involved (JI)

Justice-Involved (JI) services focus on helping people who are coming home from jail or prison get steady and supported as they re-enter the community. The goal is to reduce gaps right after release by helping the person quickly connect to care, replace confusion with a simple step-by-step plan, and remove common barriers like missing documentation, transportation issues, missed appointments, or lack of knowledge about where to go. In real-world terms, this often includes peer and community health worker support, practical goal-setting, and coordinated referrals so the person can stabilize, build momentum, and avoid getting pulled back into crisis.

Community Support (CS)

Community Supports (CS) are Medi-Cal services that help with practical needs that affect health—things that aren’t “medical treatment,” but can make or break someone’s ability to stay well. These supports can include help finding and keeping housing, short-term recovery housing after a hospital stay, medically supportive meals, home accessibility changes, respite help, and other services that reduce stress and improve stability. The big idea is simple: when basic needs are addressed, people are more likely to keep appointments, follow care plans, and avoid preventable emergencies—so the system works better for the member and the community.

These services are organized as one coordinated hub to help partners operate smoothly, connect clients to the right level of care, and keep Medi-Cal-aligned workflows sustainable. The hub supports daily program operations and systems, provides a clear referral pathway into partner-delivered clinical mental health services, and strengthens organizational readiness through onboarding, partnership guidance, and billing-focused support—so partners can move from intake to service delivery with fewer delays and less confusion.

Clinical Mental Health (Partner Services)

Clinical mental health services are provided through our clinical partner provider, Yoshida, Yamada & Washington Services, LLC, and are typically accessed through referral from a partnering Non-Profit organization or a Health Plan. These partner services include individual therapy, group therapy (including SUD and mental health groups), virtual Intensive Outpatient Programs (IOP), and CalGETS gambling addiction services. The purpose of this pathway is to make it easy for eligible clients to receive appropriate clinical care while keeping roles clear—partners refer and coordinate, and the clinical provider delivers treatment within the scope of authorization and medical necessity requirements. This ensures clean handoffs, reduced confusion for clients, and consistent coordination with the referring organization.

Non-Profit Program Management

This department supports Non-Profits and partner programs with the day-to-day structure needed to operate smoothly under Medi-Cal expectations—especially documentation flow, staff readiness, and system-based workflows. It provides practical assistance with program setup, implementation support, and problem-solving when teams run into operational barriers that slow services down. Support also includes EHR and technical coordination (access help, user training, troubleshooting, reporting issues, and data integrity checks) so staff can deliver services consistently and leadership can track outcomes and performance without system breakdowns.

We support non-profits by helping them bill for Community Health Worker (CHW) and Certified Peer Support Specialist services under Medi-Cal as a Fee-for-Service provider.
